Assessing the achievements of the Millennium Development Goals in Southern Africa
Abstract
The Millennium Development Goals (MDGs) were put forward in early 2000 and the targets are to be
reached in 2015. They are an inclusive set of well- defined goals that primarily seek to put development at
the forefront of national agendas by persuading governments to commit resources to address socio-economic
backlogs in their respective countries. Many reports have been produced on progress with The Millennium
Development Goals but certain methodologies do not accurately reflect individual country’s progress.
Governments need adequate information about their country’s performance. In this article, the performance
of Southern African countries is analyzed separately. Time series data sourced from the United Nations data
base were used to estimate parameters. The results suggest that of the six MDG goals examined, none of the
Southern African countries is poised to achieve all six goals. At best, Botswana and Swaziland could achieve
three of the goals.

Résumé
Les objectifs du Millénaire pour le développement (OMD) ont été soumises au début de 2000 et les objectifs
sont a atteindre en 2015. Ils constituent un ensemble inclusif de puits - objectifs définis qui visent
principalement a mettre le développement au premier rang des agendas nationaux en persuadant les
gouvernements a consacrer des ressources pour résoudre des retards socio-économiques dans leurs pays
respectifs. De nombreux rapports ont été produits les progrés dans Ia réalisation des OMD, mais certaines
méthodes ne reflétent pas exactement les progrés accomplis par chaque pays. Les gouvernements doivent
une information adéquate sur les performances de leur pays. Dans cet article, les performances des pays
d'Afrique australe sont analysé séparément. Données de séries chronologiques provenance de Ia base de
données des Nations Unies ont servi a estimer les paramétres. Les résultats suggérent que les six objectifs
OMD examinés, aucun de I'Afrique du Sud pays est sur le point d'atteindre tous les six objectifs. Au mieux,
Botswana et Swaziland pourraient atteindre trois des objectifs.

Analysis of demographic and health survey to measure poverty of household in Rwanda
Abstract
The use of the asset index in poverty targeting is a modern technique. We used the principal component
analysis (PCA) technique in order to create the asset index. Then the asset index was used to assess the
socio-economic status (SES) of households. The reliability of the index was tested firstly by ascertaining
whether the index was internally coherent, secondly the robustness was tested using the sub-indices such as
housing infrastructure and ownership. The methodology is applied and demonstrated using the household
survey data in Rwanda. The Rwanda data analysis showed that the age of household head, education level of
the household head, gender of the household head, place of residence, the province of household head and
size of the household (number of household members) were the significant predictors of poverty of the
household in Rwanda.

L'utilisation de lindice d’actifs du ménage dans le ciblage de la pauvreté est une technique moderne. Nous
avons utilisé |’analyse en composantes principales pour créer I’indice d’actifs du ménage. Ensuite, |’indice
dactifs du ménage a été utilisé pour évaluer le statut socio-économique des ménages. La fiabilité de l’indice
a été testée en vérifiant tout d’abord si l’indice a une cohérence interne, d’autre part la robustesse a été
testée a I’aide de sous-indices d’infrastructures de logement et de propriété. La méthode est appliquée en
utilisant les données d’enquétes auprés des ménages au Rwanda. L’analyse des données a montré qu’au
Rwanda, l’Gge du chef de ménage, son niveau d’étude, son genre, sa province, son lieu de résidence et Ia taille
du ménage étaient les facteurs prédictifs significatifs de pauvreté du ménage au Rwanda.

Trend and Determinants of Unmet Need for Family Planning Services among Currently Married Women and Sexually Active Unmarried Women Aged 15-49 in Nigeria (2003—2013)
Abstract
Data from three consecutive Nigeria Demographic and Health Surveys (2003, 2008 and 2013) were
analysed. The results show that unmet need for FP declined between 2008 and 2013 to a level less
comparable with the situation in 2003. The significant determinants of unmet need for FP included age,
marital status, education, religion, current work status, decision-making on spending personal earnings,
gender of household heads, household wealth status, number of living children (including current pregnancy),
rural-urban residence, home visit by FP workers and recent exposure to FP messages via mass media. It is
therefore necessary that FP programmers continue to develop specific responses that address the barriers to
contraceptive use.

Cette étude a analysé les données démographiques et de santé au Nigeria entre 2003 et 2013. Les résultats
montrent que les besoins en matiére de PF ont diminué entre 2008 et 2013 par rapport a 2003. Les
déterminants les plus important sont lage, l'état matrimonial, !’éducation, Ia religion, le statut de travail
actuel, Ia prise de décision dépenser les revenus personnels, le sexe du chef du foyer, [état de Ia richesse des
ménages, nombre des enfants vivant, residence rurale - urbaine, visite a domicile par les travailleurs de PF et
exposition aux messages de PF via les médias de masse. En conclusion, programmeurs PF continuent a
élaborer des réponses spécifiques qui répondent aux barriere de contraception.

Association between experience of sexual coercion and sexual behaviour: insights from the 2008 Botswana AIDS impact survey III
Abstract
Objective: Little is known about the prevalence and the effect of sexual coercion on risky sexual behaviour in
the context of Botswana. The aim of this study was to investigate the association between the experience of
sexual coercion and risky sexual behaviour.
Methods: The study used a nationally representative population-based survey data from the 2008 Botswana
AIDS Impact Survey Il]. We used cross-tabulations and multivariate logistic regression analysis to investigate
an association between the experience of sexual coercion and risky sexual behaviour.
Results: Out of the 2,455 respondents, the prevalence of sexual coercion was 5% (3.1% males and 8.1%
females). The adjusted odds ratios showed that only the respondents who reported having experienced sexual
coercion compared to those who did not were 1.9 times more likely to report having engaged in multiple
sexual partnerships and this relationship was statistically significant at 1% level.
Conclusion: The results suggest that in order to implement effective HIV/AIDS prevention strategies it is
imperative to adopt a more holistic and multifaceted approach that includes issues of sexual violence.

Objectif: On connait la prévalence et I'incidence de Ia coercition sexuelle sur les comportements sexuels a
risque dans le contexte du Botswana. Cette étude visait a examiner le lien entre l'expérience de Ia coercition
sexuelle et les comportements sexuels a risque.
Méthodes: L'étude a utilisé une données représentatives au niveau national axée sur la population de Ia
2008 Botswana AIDS Impact Survey Ill. Nous avons utilisé des croisés et l'analyse de régression logistique
multivariée pour enquéter sur une association entre I'expérience de fa coercition sexuelle et les
comportements sexuels d risque.
Résultats: Sur les 2 455 répondants, la prévalence de Ia coercition sexuelle était de 5 % (3,1 % hommes et
8,1 % des femmes). Les rapports de cotes ajustés ont montré que seulement les répondants qui déclarent
avoir subi Ia coercition sexuelle par rapport a ceux qui n'ont pas étaient 1,9 fois plus susceptibles de déclarer
avoir entrepris des partenariats sexuels multiples et cette relation était statistiquement significative au
niveau de | %.
Conclusion: Les résultats suggérent que pour mettre en ceuvre des stratégies efficaces de prévention du
VIH/sida il est impératif d'adopter une approche plus holistique et aux multiples facettes qui comprend des
questions de violence sexuelle.

Projecting population, numbers of households and dwelling units in South Africa 2011-2021
Abstract
Future trends in population, number of households and dwelling units are critical components in strategic
planning for future service delivery in South Africa. To aid planning therefore, this study analysed provincial
trends in fertility, mortality and net migration as well as trends in the number of households and dwelling units.
On the basis of the trends and assumptions about the future, households and dwelling units were projected for
the period 201 1-202! using component functional models and ratio method. The results suggest that the size
of the population aged 15 years and over in South Africa may increase from 36.4 million in mid-201 1 to 42.5
million in mid-202 1. Gauteng province is projected to have the largest number of persons in this age group.
The number of households nationally is projected to increase from 15 million in mid-20 | | to 20 million by mid-
2021 with Gauteng having the highest number of households.

Les tendances futures dans la population, nombre de ménages et unités résidentielles est des composantes
essentielles dans la planification stratégique pour la prestation des services futurs en Afrique du Sud. Pour
faciliter la planification par conséquent, cette étude a analysé tendances provinciales en fécondité, mortalité
et migration nette ainsi que les tendances du nombre de ménages et de logements. Compte tenu des tendances
et des hypothéses quant a l'avenir, ménages et unités résidentielles ont été prévues pour la période 201 1-202 1
a l'aide de modéles fonctionnels de composants et de la méthode du rapport. Les résultats suggérent que la
taille de la population Ggée de 15 ans et plus en Afrique du Sud peut passer de 36,4 millions a la mi-2011 a
42,5 millions en milieu-202 1. La province de Gauteng devrait pour avoir le plus grand nombre de personnes
dans ce groupe d'age. Le nombre de ménages a I'échelle nationale devrait pour augmenter de 15 millions a la
mi-2011 @ 20 millions d'ici mi-2021 avec Gauteng ayant le plus grand nombre de ménages.

Female genital mutilation (FGM) and sexual functioning of married women in Oworonshoki Community, Lagos State, Nigeria
Abstract
This article examines the nexus between Female Genital Mutilation (FGM) and three thematic/fundamental
areas of women sexual functioning in the study location. These thematic areas are women’s sexual
desire/initiation, sexual activity and sexual satisfaction/enjoyment. To achieve the objectives of the study, a
cross-sectional survey research method was adopted and 350 questionnaires were administered among ever
married women. Elicited data were analyzed with the aids of quantitative analytical techniques. Findings of
the study reveal that FGM hampered effective and efficient sexual functioning of the women in the study
location as circumcised women reported defective sexuality and sexual dysfunctional. Based on these findings,
it has been recommended that both state and non-state actors working in the areas of women’s reproductive
health should develop appropriate intervention programmes to reduce/eradicate the practice of FGM in the
study location. This will invariably help the married women to reach orgasm during sexual intercourse.

Cet article examine le lien entre les mutilations génitales féminines (MGF) et trois domaines
thématiques/fondamentaux des femmes fonctionnement sexuel sur le lieu de |'étude. Ces domaines
thématiques sont le désirfinitiation, l'activité sexuelle sexuelle des femmes et sexuelle satisfaction/plaisir.
Pour atteindre les objectifs de ['étude, une méthode de recherche de I'enquéte transversale a été adopté et
350 questionnaires ont été administrés chez les femmes mariées. Des données a suscité ont été analysées
avec les aides de techniques d'analyse quantitative. Les résultats de |'étude révélent que les MGF entravé le
fonctionnement sexuel efficace et efficiente des femmes dans le lieu de [étude que les femmes excisées ont
signalé la sexualité défectueux et dysfonctionnement sexuel. Basé sur ces résultats, il a été recommandé que
les acteurs étatiques et non étatiques qui travaillent dans les domaines de la santé reproductive des femmes
devraient élaborer des programmes d'intervention appropriées pour réduire/éliminer la pratique des MGF
dans le lieu de étude. Ce sera toujours d'aider les femmes mariées a atteindre lorgasme pendant les
rapports sexuels.

Nurses’ International Migration and the Crystallizing “Culture of Exile’ in Nigeria: Historical Trends, Dynamics and Consequences
Abstract
This paper examines an oft-neglected feature of international migration: social changes (disruptions and/or
dislocations of their normal ways of living) in source societies in response to the exigencies of these
migrations, and their eventual consequences. {t demonstrates how policies in developed economies
inadvertently impact on developing societies by creating new social conditions. An exploratory case of
Nigerian nurses’ international migration, the paper takes the hypothetical perspective that the socio-
economic effects of migration have engendered a crystallizing ‘culture of exile’ among significant youth
populations. It examines the lures/motivation of nurse training and establishes a new dominant drive — the
urge to migrate to developed, high-income economies. This development is a direct result of the long-term
‘progressive impact’ of migrated nurses’ remittances in local societies. The paper shows how locals’ belief in
the high probability of nurses to attract ‘overseas’ suitors/spouses also feeds into the narrative. The
development dire consequences were also highlighted.

Ce document examine une caractéristique souvent négligé de 1a migration internationale: les changements

politiques dans les économies développées incidence par inadvertance sur les sociétés en développement en

internationale, le papier prend la perspective hypothétique que les effets socio-économiques des migrations

Maternal health care in Nigeria: Do community factors moderate the effects of individual-level Education and Ethnic origin?
Abstract
Maternal mortality ratio in Nigeria is still high despite the initiation of the safe motherhood initiative. The
poor maternal health situation has been attributed to inadequate use of maternal health care services. This
study examines whether community factors moderated the association between individual factors and the use
of skilled ANC. Data on 17560 women aged [5-49 years drawn from 2008 Nigeria Demographic and Health
Survey were used. Multilevel logistic regression models were used for analysis. Results reveal variations in the
use of skilled ANC across the Northern and Southern regions. Residence in communities with a high
proportion of women that delivered in a health facility increased the odds of skilled ANC utilization.
Community education and poverty moderated the association between individual factors and the utilization of
skilled ANC. To improve the use of ANC and increase the pace towards achieving the sustainable
Development Goal-improved maternal health in the post 2015 era, increasing health facility delivery,
women’s education and targeting poverty alleviation programs in disadvantaged communities should be taken
into consideration.

Ratio de mortalité maternelle au Nigéria est encore élevé malgré l'ouverture de I'initiative pour une maternité
sans risque. La mauvaise santé maternelle situation a été attribuée a I'insuffisance des services de soins de
santé maternelle. Cette étude examine si facteurs communautaires a modéré l'association entre facteurs
individuels et [utilisation de 'ANC qualifiés. Données sur 17560 femmes Ggées de 15 a 49 ans tirées de 2008
le Nigeria enquéte sur la démographie et Ia santé ont été utilisés. Multiniveau modéles de régression
logistique ont été utilisées pour I'analyse. Les résultats révélent des variations dans [utilisation de [ANC
qualifiés partout dans les régions du Nord et du Sud. Résidence dans des communautés avec une forte
proportion de femmes que livré dans un établissement de santé a accru les chances de I'ANC qualifiés
lutilisation. L'éducation communautaire et la pauvreté modérée |'association entre facteurs individuels et
lutilisation de [ANC qualifiés. Pour améliorer [utilisation de 'ANC et de I'augmentation du rythme vers Ia
réalisation de lobjectif de développement durable et I'amélioration de la santé maternelle dans le poste 2015
ére, d accroitre la santé installation de livraison, 'éducation des femmes et le ciblage des programmes de

Statistical coherence of primary schooling in population census microdata: IPUMS-International integrated samples compared for fifteen African countries
Abstract
The IPUMS-International project, now in its fifteenth year, integrates and disseminates population microdata
for twenty-two African countries (82 countries world-wide) and the number continues to increase as more
National Statistical Offices cooperate with the initiative. Statistical quality is a serious concern both for the
producers of the microdata as well as the researchers who use them. This paper applies the intra-cohort
comparison method to pairs of integrated (harmonized) samples for fifteen African countries to assess
statistical coherence using as a benchmark the proportion completing primary school by single years of birth.
Samples for six countries show near perfect coherence (R2 > .9, and regression coefficients ~1.0 +/-
0.6 <0.9). Large
deviations from 1.0 characterize samples for only four countries. On the whole, the results suggest that
samples for the fifteen countries have considerable utility for socio-demographic analysis.

Le projet IPUMS-International entre dans sa quinziéme année, et a pour but de préserver, d’harmoniser
(intégrer) et de disséminer les données individuelles des recensements de population. Actuellement il couvre
les données de 22 pays africains et de 82 pays a travers le monde. Le nombre de pays couverts augmente
chaque année, au fur et ad mesure que les instituts nationaux de statistique acceptent de collaborer a cette
initiative. L’évaluation de la qualité des données reste une question importante, tant que pour les
producteurs que pour les utilisateurs des données individuelles. Cet article utilise la méthode de comparaison
intra-cohortes entre des couples de données harmonisées (intégrées) d’échantillons de recensement de [5
pays africains pour évaluer leur cohérence statistique, en prenant l’exemple de !a proportion de personnes
ayant terminé I’école primaire, classées par année de naissance. Dans un premier groupe de 6 pays, on
trouve une cohérence presque parfaite entre les recensements successifs (R2 > 0,9; coefficients de
régression b ~ 1,0 0,08). Dans un second groupe de 5 pays, les coefficients sont un peu plus faibles, mais
restent acceptables (R2 > 0,6; b<0,9). Les écarts a l'unité plus marqués se trouvent dans les échantillons
des quatre derniers pays. Dans l’ensemble, les résultats de cette analyse montrent que ces échantillons sont
fiables et peuvent étre utilisés pour !’analyse sociodémographique.

Support for Voluntary Medical Male Circumcision (VMMC) for HIV Prevention among Men and Women in Zimbabwe
Abstract
Background: Medical male circumcision was introduced in Zimbabwe in 2009 as an additional HIV
prevention method. This study sought to investigate support for the roll-out of voluntary medical male
circumcision (VMMC) and men’s willingness to get circumcised for HIV prevention.
Data and Methods: Data for this study was collected from a randomly selected sample of 681 men and
women in the age group | 8-49 years in Harare, Zimbabwe. The obtained data was analysed using descriptive
statistics, bivariate and regression analysis.
Results and Conclusion: The results of the study suggest that knowledge and acceptability of VMMC is
high. However, despite the relatively high knowledge and acceptability of VMMC, less than half of the male
participants were willing to undergo circumcision for HIV prevention. The study concluded that there is an
apparent gap between knowledge and acceptability of VMMC and men’s willingness to undergo circumcision
for HIV prevention.

Résumé
Contexte: La circoncision masculine médicale a été introduite au Zimbabwe en 2009 comme méthode
préventive additionnelle du VIH. L’étude examine les conditions de déploiement de cette méthode et Ia
volonté des hommes de se faire circoncire médicalement pour prévenir le VIH.
Données et méthodes: Les données de |’étude proviennent d’un échantillon aléatoire de 681 hommes et
femmes du groupe d’Gges 18-49 ans vivant a Harare, Zimbabwe. Des analyses descriptives bi-variées et une
analyse de régression ont été appliquées aux données.
Résultats et conclusion: Les résultats de l'étude indiquent une connaissance et une acceptation élevées de
Ia circoncision masculine. Malgré cela, seulement moins de la moitié des hommes enquétés ont manifesté le
désir de se faire circoncire pour une prévention du VIH. L’étude aboutit a la conclusion qu’il existe un écart
entre Ia connaissance, l’acceptabilité de la circoncision et la volonté des hommes de subir cette opération.

Leveraging mobile technology to reduce barriers to maternal, newborn and child health care: a contribution to the evidence base
Abstract
Significant progress has been
achieved worldwide in recent decades to
improve the health of women and
reduce the number of children who die
before their fifth birthday (UN 2010).
The efforts of governments in low and
middle income countries (LMICs) and
their development and funding partners
have coalesced around the twin
Millennium Development Goals (MDGs)
4 and 5, and steadily contributed to
breaking the vicious cycle of poverty and
poor maternal, newborn and child health
(MNCH), even in some of the poorest
countries in the world (WHO & UNICEF
2012). In 2013, the world’s maternal
mortality ratio (MMR) was estimated at
210 deaths per 100,000 live births, down
from 380 in 1990. Despite this progress,
more than 275,000 women worldwide
die annually from preventable
complications related to pregnancy and
childbirth. Most of these deaths occur in
sub-Saharan Africa (62%) and Southern
Asia (24%) (UN 2014). Another major

Fostering the use of quasi-experimental designs for evaluating public health interventions: insights from an mHealth project in Malawi
Abstract
The evidence base to support the growing field of mHealth is relatively nascent,
with most studies lacking the level of rigor needed to inform scale up of
interventions. This paper investigates the impact of a maternal, newborn and child
health (MNCH) mHealth project in Malawi, comparing the intention-to-treat
(ITT) and the treatment on the treated (TOT) estimates, and discussing the
implications for future evaluations. Services offered included a toll-free case
management hotline and mobile messaging service for women and children. The
evaluation methods included a quasi-experimental pre-test post-test design,
consisting of cross-sectional household surveys. A total of 4,230 women were
interviewed in the intervention area and 2,463 in the control site. While the
intervention did not have any ITT effects of the MNCH outcomes studied, there
were large TOT effects. Rigorous evaluation designs can be successfully applied to
mHealth pilot projects, helping to understand what works and what does not.

Where there is no phone: The benefits and limitations of using intermediaries to extend the reach of Health to individuals without personal phones in Malawi
Abstract
The purpose of this study is to identify the benefits and limitations associated with
intermediaries to provide access to and increase utilization of an mHealth
intervention amongst people without personal phones in Balaka District, Malawi.
A mixed-methods approach was utilized including quantitative data on usage and
focus groups and interviews with users and volunteers. Community volunteers
equipped with mobile phones served as intermediaries and were critical access
points to the service for users without personal phones. However, there were
challenges maintaining phones and solar panels, sustaining volunteer motivation
and understanding how to use the service. While these strategies had a number of
limitations, the majority of users (more than 65%) were individuals without a
personal phone, who may not have been able to access the service otherwise.
Further research is needed to better understand the resources, strategies and
effort needed to sustain access through intermediaries in the long-term.

Résumé
L’objectif de cette étude est d’identifier les bénéfices et les limites de Putilisation
des intermédiaires pour permettre l’accés et améliorer [utilisation d’une
intervention mSanté chez les personnes n’ayant pas de téléphone personnel dans
le district de Balaka au Malawi. Une approche mixte a été utilisée : des données
quantitatives sur l'utilisation du téléphone ont été collectées, et des groupes de discussion et entrevues avec des usagers et des volontaires ont été organisés. Les
volontaires équipés d’un téléphone mobile dans !a communauté ont servi
d'intermédiaires et ont été des points d’accés critiques au service pour les
personnes n’ayant pas de téléphone personnel. Cependant, maintenir les
téléphones et les panneaux solaires, soutenir la motivation des volontaires, et
comprendre comment utiliser le service ont été de véritables défis. Bien que toutes
ces stratégies présentent certaines limites, la majorité d’utilisateurs (plus de
65%) étaient des personnes n’ayant pas de téléphone, et qui n’auraient pas pu
accéder qutrement aux services offerts. D’autres recherches sont nécessaires pour
mieux comprendre les ressources, les stratégies et les efforts requis pour soutenir
l’accés au service a long terme par l'utilisation des intermédiaires.

Improving care-seeking for facility-based health services in a rural, resource-limited setting: Effects and potential of an mHealth project
Abstract
The aim of this paper was to investigate the impact of a toll-free hotline and
mobile messaging service on care-seeking behaviors. Due to the low uptake of the
services, the treatment on the treated estimate is used. For maternal health, the
intervention had a strong, positive impact on antenatal care initiation and skilled
birth attendance. No effect was observed for postnatal check-ups, receiving the
recommended four antenatal care visits and vitamin A uptake. A negative effect
was observed on tetanus toxoid coverage. For child health, no change was seen in
child immunization, and a significant decrease was observed for care-seeking for
children with fever. Different factors are associated with care-seeking, which may
explain in part the variations seen across care-seeking behaviors and possible
influence of exogenous factors. Introduction of mHealth services for demand
generation require attention to local health systems to ensure adequate supply
and quality are available.

L’objectif de cet article est d’analyser l’impact sur les comportements de demande
de soins, d’un projet mSanté consistant en un service de ligne téléphonique sans
frais et de messagerie mobile. En raison de la faible utilisation, l’estimateur du
traitement sur les traités est utilisé. L’intervention a eu un impact positif sur
Pinitiation aux soins prénataux et l’accouchement par un personnel qualifié. Aucun
effet n’a été observé sur les consultations post-natales, le respect de quatre visites
prénatales ou utilisation de la vitamine A. Un effet négatif a été observé dans la
couverture antitétanique. Pour Ila santé infantile, aucun changement n’a été
observé sur la vaccination, alors qu’une baisse significative est observée sur le
traitement des enfants ayant souffert de !a fiévre. Différent facteurs sont associés
a la demande des soins, ce qui expliquerait en partie les variations observées dans
les comportements de demande des soins. Pour l’introduction des services mSanté,
une attention doit étre portée au systéme de santé, afin de s’assurer que
’approvisionnement et Ia qualité des soins sont adéquats

Strengthening the home-to-facility continuum of newborn and child health care through mHealth: Evidence from an intervention in rural Malawi
Abstract
This paper assesses the impact of a mobile health (mHealth) project on uptake of
home-based care for newborn and child health, and investigates the extent to
which uptake of home-based care resulted in lessened pressure on health facilities
for conditions that can be handled at the household level. It uses mixed methods
consisting of cross-sectional household surveys data from a quasi-experimental
pre-test post-test design as well as qualitative data. The results show a large,
positive effect of the project on the aggregate home-based care for child health,
and a sharp, negative impact on facility-based care seeking for fever among
children whose mothers/caretakers used the services offered by the intervention.
Reasons for using the services mainly relate to the potential of avoiding
unnecessary trips to the health facility for care that could be provided at home.
The project provides insights on mHealth and community-based programming to
improve newborn and child health care delivery.

Cet article analyse impact d’un projet de santé mobile (mSanté) sur utilisation
des soins a domicile en santé néonatale et infantile, et examine dans quelle
mesure Iutilisation des soins a domicile réduirait la pression exercée sur les
établissements de santé pour les cas qui peuvent étre administrés a domicile. I]
utilise des données quantitatives collectées a partir des enquétes-ménages
transversales basées sur une méthode quasi-expérimentale avec comparaison
avant-aprés, et des données qualitatives. Les résultats indiquent un effet positif enégatif trés prononcé sur les soins aux centres de santé pour Ia fiévre, parmi les
enfants dont les méres/gardiennes ont utilisé les services offerts par I’intervention.
Eviter les voyages non-nécessaires vers les établissements de santé lorsque les
soins peuvent étre administrés a domicile constitue l'une des raisons majeures de
cet effet négatif: Le projet fournit une lumiére concernant le domaine mSanté
dans le cadre communautaire visant a améliorer la prestation des soins de santé
néonatale et infantile.

Application of mHealth to improve service delivery and health outcomes: Opportunities and challenges
Abstract
The use of mobile technologies for health related activities (mHealth) is a new but
rapidly progressing activity with global penetration. However, few programs have
been implemented at scale. The objective of this paper is to review the
background and evidence on mHealth, particularly with respect to the benefits
and challenges of scale-up. A comprehensive review of literature on mHealth,
aspects of eHealth and the related regulatory environment was undertaken in
August 2014. mHealth innovations vary broadly in purpose, delivery channel and
target population. As a result of its broad applicability and geographic reach, the
benefits of successfully, safely and widely implemented mHealth are numerous
and promising. However, these benefits can only be realized if the associated risks
are minimized and the barriers are purposefully overcome. Government
stewardship and leadership is crucial in overcoming these barriers and scaling up
mHealth.

L'utilisation des technologies mobiles pour les activités de santé (mSanté) est une
activité récente mais en nette et rapide croissance. Cependant peu de
programmes ont été mis en place a grande échelle. L’objectif de cette étude est
d’examiner le contexte et les évidences dans le domaine de mSanté,
particuliérement en ce qui concerne les bénéfices et les défis de son expansion.
Une revue de littérature compréhensive sur mSanté, les aspects d’eSanté, et le
mécanisme de régulation a été menée en Aotit 2014. Les innovations de mSantévarient largement par rapport aux objectifs poursuivis, les méthodes de prestation
de service et la population-cible. En raison de sa large applicabilité et sa portée
géographique, les bénéfices des projets exécutés avec succés et a grande échelle,
et garantissant la sécurité, sont nombreux et prometteurs. Cependant, ces
bénéfices ne peuvent étre réalisés que si les risques associés sont minimisés et les
barriéres surmontées. Une bonne gouvernance et un réle-clé des gouvernements
sont cruciaux pour surmonter ces barriéres et étendre le domaine de mSanté.

Accelerating and improving survey implementation with mobile technology: Lessons from PMA2020 implementation in Lagos, Nigeria
Abstract
Large-scale nationally representative surveys have traditionally been implemented
using paper surveys, necessitating secondary steps of data entry and management
after data collection. Errors occurring during data collection or entry may not be
rapidly identified. The Performance Monitoring and Accountability (PMA20207)
project implementation in Lagos, Nigeria demonstrates four advantages to
integrating mobile technology into survey implementation. First is the rapidity of
data collection; data collection lasted six weeks from mapping/listing to final
collection — and, since completed surveys are uploaded to a cloud-based server,
identification of errors can occur in near real-time. Second, time-stamping and
GPS marking allow for improved quality assurance. Third, the inclusion of GPS
coordinates creates new opportunities to analyze relationships of distance with
use of health services. Fourth, PMA2014/Lagos utilized a 10% resample of
households to validate data collection allowing for rapid identification of
questionable data and quality control.

Traditionnellement, les enquétes a grande échelle représentatives qu niveau
national sont effectuées en version papier, ce qui nécessite des étapes secondaires
comme Ia saisie et la gestion de données aprés le recueil. Il se peut que les erreurs
qui se présentent pendant Ia collecte ou la saisie de données ne soient pas
identifiées de fagon rapide. La mise en ceuvre du projet « Performance Monitoring
and Accountability » (PMA2020') a Lagos au Nigeria démontre quatre avantages
de I’intégration de la technologie mobile dans la réalisation des enquétes. Le premier avantage est le temps de terrain rapide ; le recueil de données s’est
effectué pendant six semaines, de la phase de cartographie et recensement des
ménages jusqu’a Ia fin de la collecte. De plus, puisque les enquétes complétées
sont téléchargées au serveur basé dans les nuages, identification des erreurs peut
se passer pratiquement en temps réel. Deuxiémement, I’horodatage et le
géoréférencement en utilisant la fonction GPS permettent une assurance qualité
améliorée. Troisitmement, linclusion des coordonnées GPS crée des nouvelles
opportunités pour analyser lassociation entre la distance et utilisation des
services de santé. Quatriémement, PMA2014 a Lagos réinterroge 10% des
ménages pour Ia validation du recueil de données, ce qui permette l’identification
rapide des données non valables et un contréle qualité.

Applying Human Rights-Based Approaches to Public Health: Lessons Learned from Maternal, Newborn and Child Health Programs
Abstract
The application of human rights norms at the national policy level is largely
codified, but rigorous research on the field-level application of a human rights-
based approach (HRBA) to health programs is still in its infancy. The paper
identifies human rights norms from international law and standards that are
relevant to maternal, newborn and child health (MNCH) programs, and presents
an example of how a HRBA has been previously applied in an MNCH project. It
introduces a HRBA framework recently developed by Concern Worldwide and
partners, and conducts a document analysis of a previous Concern Worldwide
project in Malawi. Discussion focuses on where and how the project could have
benefited from incorporation of a HRBA in project design, implementation and
evaluation. The authors conclude by discussing how such inclusion could have
impacted project outcomes, and how such analysis can help inform future efforts
to implement a HRBA to health.

politiques nationales est largement codifié, mais les recherches rigoureuses sur
application de !’approche basée sur les droits de homme (ABDH) dans les
programmes de santé est encore embryonnaire. Cet article identifie les normes en
matiére des droits de homme a partir du droit international qui sont pertinents
pour les programmes de santé maternelle, néonatale et infantile (SMNI), puis
présente un exemple de |’application de l'ABDH dans un projet de SMNI. II
indicateurs du projet, et comment une telle analyse permet d’informer les efforts
futurs pour intégrer ! ABDH en santé.

A Look Back to Look Forward: A Multidisciplinary Evaluation of an mHealth Service in Malawi
Abstract
Prioritized as an __ innovative
opportunity to capitalize on the
unanticipated or unprecedented access
to telecommunications and computing
power, mHealth is being applied at
increasing scale and scope to facilitate
attainment of the Millennium
Development Goals (MDGs) for health.
It is already being explored as a strategy
for the upcoming Sustainable
Development Goal (SDG) focus on
Universal Health Coverage (Mehl &
Labrique 2014). The special issue on
mHealth for maternal, newborn, and
child health (MNCH) of the African
Population Studies Journal provides an
important contribution to the growing
evidence in an emergent field. Exploring
the use and impact of mobile technology
for MNCH primarily in Malawi, this
collection of papers range from
presentations of results from impact
evaluations to exploration of equity and
human rights approaches to mHealth
and highlights many of the opportunities
and challenges of the present day and
those we ought to address in the future.
It illustrates the growing trend towards
assessing the impact of mHealth for
MNCH through the measurement of
standard indicators for MNCH, applying

more rigorous experimental and quasi-
experimental designs with statistically
significant sample sizes and more
advanced exploration of equity, human
rights based approaches, and emerging
policy issues. It also helps to fill in some
critical gaps in the evidence base
(Philbrick 201 3).
mHealth is often touted for its ability
to increase access to and coverage of
services, facilitate task shifting services
from facilities to communities to
households, generate efficiencies, and
improve quality of care. It is increasingly
included as a strategy in prominent
global initiatives including Every Woman
Every Child, the Child Survival Call to
Action, the Every Newborn Action Plan
and the UN Commission on Life-Saving
Commodities for Women and Children’s
Health. Each paper sheds light on an
important aspect of mHealth in ways
that increase accountability and inform
national scale up as well as the broader
mainstreaming of mHealth into health
systems and domains.
